Medication Reconciliation (MR) is an essential process of obtaining a complete and accurate list of a patient’s current home medications and comparing a prescriber’s admission, transfer and discharge order to that list. MR has been identified as one of the major areas of clinical development by the HA Pharmacy Coordinating Committee and more pharmacists may be recruited to fulfill this role. We have invited one of the pioneer pharmacists of MR in Hong Kong, Ms. Ritchie Kwok, to speak to us on her experience in this field. | ||
