Medication Reconciliation (MR) is an essential process of obtaining a complete and accurate list of a patient’s current home medications and comparing a prescriber’s admission, transfer and discharge order to that list. MR has been identified as one of the major areas of clinical development by the HA Pharmacy Coordinating Committee and more pharmacists may be recruited to fulfill this role. We have invited one of the pioneer pharmacists of MR in Hong Kong, Ms. Ritchie Kwok, to speak to us on her experience in this field.

The knowledge of how foreign healthcare systems works provides us with alternative perspectives to look into our own, and could inspire us to do things differently and more effectively. The second part of the Forum brings us to the UK, where Mr. Stephen Man will describe to us the role of the pharmacist in different sectors of the British healthcare system, focusing on clinical pharmacy development and illustrated with case studies.
