Notes:
1. Participants must comply with the latest COVID-19 vaccination requirements and relevant regulations imposed by the Hong Kong Government.

3. If you have already sent the AGM reply slip back to us by email, you DO NOT need to fill in the online registration form again.
STEP-UP PRECAUTIONARY MEASURES FOR PHYSICAL PARTICIPATION
1. Due to the deteriorating COVID-19 situation in Hong Kong, please note that hot meals will not be served on-site after the AGM. Instead, packed meals will be provided to all participants after the meeting. Eating and Drinking are not allowed inside the venue.
2. All participants are required to scan the LeaveHomeSafe (LHS) mobile app upon arrival and to ensure the vaccination status complies with the Vaccine Pass arrangement of HKSAR Government.
3. Participants MUST do the Rapid Antigen Test (RAT) on the day of the event. Participants will be asked to present proof of a negative rapid antigen test (RAT) result obtained within 24 hours before entering the venue. (Please take photo of the test result as proof)
Dear Members of the SHPHK,
The 35th Annual General Meeting will be held on 15th July 2022 (Friday) at Function Room 4-8, 3/F, The Mira Hong Kong, 118 Nathan Road, Tsim Sha Tsui.
To celebrate the 35th anniversary of SHPHK, a lecture on “COVID-19 Pre-Exposure Prophylaxis for Immunocompromised Patients" presented by Dr. Anthony Raymond Tam, Associate Consultant and Infectious Disease Specialist, Department of Medicine, Queen Mary Hospital and Mr. Vincent Wong, General Committee Member of SHPHK, will be held at 7:00pm before the AGM.
Dinner will be served afterwards.
